Job summary

Fuze Health Inc. seeks a Registered Nurse to support the telehealth care team by providing remote outreach to Medicare members and ensuring compliance with Stars measures. The role also confirms ED discharge and may close other HEDIS measures while delivering compassionate virtual care.

Schedule: Mon-Fri 12:30pm-9pm EST and one Saturday per month 9am-5:30pm. Strong emphasis on patient education and documentation, with startup environment experience preferred.

Qualifications

• BSN required and active, non-restricted nursing license.
• 2+ years as a Registered Nurse.
• 1-2 years in remote healthcare or telehealth.
• Strong computer skills and EMR familiarity.
• Excellent verbal and written communication.

Responsibilities

• Provide remote telehealth patient care and support.
• Conduct virtual assessments and review medical histories.
• Accurately document patient conditions and care interactions.
• Educate patients and families on conditions and medications.
• Collaborate with care teams to develop and implement care plans.
• Maintain regulatory compliance and protect patient privacy.
• Engage in ongoing education and stay updated on telehealth best practices.
• Demonstrate compassionate communication in all interactions.
