Job summary

Fuze Health Inc. is seeking a Registered Nurse to support telehealth outreach for Medicare members. The RN will conduct remote assessments, document care, educate patients, and collaborate on individualized care plans from a centralized operations team. This full-time role requires a BSN and active compact state license; schedule includes afternoons with occasional Saturday coverage.

Qualifications

• Bachelor of Science in Nursing (BSN) required.
• Active, non-restricted compact state nursing license.
• 2+ years RN experience.
• 1-2 years remote healthcare/telehealth experience.
• Strong computer skills and EMR familiarity.
• Clear communication in verbal and written form.
• Ability to follow SOPs in fast-paced environments.
• Strong customer service and interpersonal skills.

Responsibilities

• Provide remote telehealth patient care and support.
• Conduct virtual patient assessments and review histories.
• Document patient conditions, histories, and care interactions.
• Educate patients and families on conditions, medications, and self-care.
• Collaborate with healthcare providers to develop care plans.
• Maintain regulatory compliance and accurate records.
• Engage in ongoing education on telehealth practices.
• Demonstrate compassionate and professional communication.
