Fuze Health Inc. is hiring a Registered Nurse for telehealth patient care, focusing on Medicare members and ensuring adherence to stars measures and ED discharge follow-ups, while delivering high-quality virtual care.

Requirements include a BSN, active multi-state RN license, 2+ years RN experience, and 1-2 years in remote/telehealth settings. Schedule: Mon–Fri with one Saturday per month, supporting a patient-centered care team.

Qualificações

• BSN degree is required.
• Active, non-restricted nursing license with primary state of licensure among listed states.
• 2+ years of experience as a Registered Nurse.
• 1-2 years of experience in a remote healthcare/telehealth setting.
• Strong computer skills and familiarity with EMR systems.
• Strong communication and teamwork skills.

Responsabilidades

• Provide remote patient care and support through telehealth platforms.
• Conduct virtual patient assessments, including reviewing medical histories and current symptoms.
• Accurately assess and document patient conditions, histories, and care interactions.
• Educate patients and their families on medical conditions, medication adherence, and self-care practices.
• Work closely with other healthcare providers to develop and implement individualized care plans.
• Maintain accurate records and ensure adherence to healthcare regulations and privacy laws.
• Stay up to date with medical advancements, telehealth protocols, and best practices.
• Demonstrate empathy, professionalism, and effective communication in all patient interactions.
