Job summary

Fuze Health in the United States is seeking a Registered Nurse to support the FuzeRx Central Operations telehealth care team, providing remote outreach to Medicare members and ensuring compliance with a new Stars measure while confirming ED discharge.

You will conduct virtual assessments, document conditions, educate patients and families, and collaborate with the care team to develop individualized care plans while maintaining privacy and regulatory standards.

Qualifications

• B SN degree required and active, non-restricted license in a compact state.
• 2+ years of RN experience; remote/telehealth preferred.
• Strong computer skills and EMR familiarity; excellent customer service.

Responsibilities

• Provide remote telehealth patient care to Medicare members.
• Conduct virtual assessments and document patient conditions.
• Educate patients and families on conditions, adherence, and self-care.
• Collaborate with care team to develop care plans.
• Ensure regulatory compliance and privacy.
