Job summary

Fuze Health Inc. is seeking a Registered Nurse to support the FuzeRx Central Operations telehealth care team by performing remote outreach to Medicare members and confirming adherence to a new Stars measure, with a focus on safe, high-quality virtual care.

Key duties include telehealth patient care, virtual assessments, medical documentation, patient education, and collaboration on care plans, while ensuring regulatory compliance and strong communication.

Qualifications

• BSN degree and active nursing license in one of the US states.
• Minimum 2 years of experience as a Registered Nurse.
• 1-2 years in a remote healthcare or telehealth setting.
• Strong computer skills and familiarity with EMR systems.
• Clear verbal and written communication.
• Ability to follow SOPs in a fast-paced environment.

Responsibilities

• Telehealth patient care: provide remote patient care and support through telehealth platforms.
• Virtual assessments: conduct virtual patient assessments, including reviewing medical histories and current symptoms.
• Medical documentation: accurately assess and document patient conditions, histories, and care interactions.
• Patient education: educate patients and families on medical conditions, medication adherence, and self-care practices.
• Care plan collaboration: work with other healthcare providers to develop and implement individualized care plans.
• Regulatory compliance: maintain accurate records and ensure adherence to healthcare regulations and privacy laws.
• Continuous education: stay up to date with medical advancements, telehealth protocols, and best practices.
• Compassionate communication: demonstrate empathy, professionalism, and effective communication in all interactions.
