Job summary

Fuze Health is seeking a Registered Nurse to support the telehealth care team by remotely assisting Medicare members and ensuring compliance with Stars measures. The role focuses on post-discharge outreach and ongoing care coordination in a patient-centered environment.

As part of a full-time telehealth schedule, you will conduct virtual assessments, document clinical information, educate patients and families, and collaborate with providers to optimize care plans while adhering to privacy

Qualifications

• BSN degree and active, unrestricted nursing license.
• 2+ years of RN experience; telehealth backgrounds preferred.
• Strong computer skills and EMR familiarity.
• Excellent customer service and collaboration in high‑volume settings.
• Ability to work independently within a care team and comply with privacy laws.

Responsibilities

• Telehealth patient care and remote outreach for Medicare members.
• Conduct virtual assessments reviewing medical histories and symptoms.
• Document patient conditions, histories and care interactions accurately.
• Educate patients and families on conditions, meds, and self‑care.
• Collaborate with providers to develop and implement care plans.
• Maintain regulatory compliance and privacy standards.
• Pursue ongoing education on telehealth protocols and best practices.
