Qualifications

PharmD or Bachelor of Pharmacy required.

Responsibilities

Deliver telephonic or video-based medication counseling and education to patients and caregivers.
Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
Conduct medication reconciliations and reviews for high-risk or complex patients.
Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
Document interventions and communications according to organizational standards.
Support population-health and disease-management programs.
