Qualifications

• PharmD or Bachelor of Pharmacy required.
• Active U.S. pharmacist license required (multi-state licenses preferred).
• Ambulatory care/MTM or clinical experience preferred.

Responsibilities

• Deliver telephonic or video-based medication counseling to patients and caregivers.
• Monitor therapy outcomes and adherence; recommend optimizations as needed.
• Conduct medication reconciliations and reviews for high-risk patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document interventions and communications per standards.
• Support population-health and disease-management programs.
