Qualifications

• Licensure: Active U.S. pharmacist license (multiple state licenses preferred).
• Ambulatory care/MTM or clinical experience preferred; hospital/community pharmacists welcome.

Responsibilities

• Deliver telephonic/video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations.
• Conduct medication reconciliation and reviews for high-risk patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document interventions and communications per organizational standards.
• Support population-health and disease-management programs.
