Job summary

Blue Circle Health is seeking a dedicated Type 1 Diabetes Support Guide to assist participants through a remote program and coordinate care with the care team.

You will educate participants, provide empathetic support, and help setup virtual visits using Zoom and our EHR system. This full-time, remote role requires strong communication, healthcare experience, and a compassionate approach to diverse participants across the United States.

Qualifications

• Must have lived experience with Type 1 Diabetes or close familiarity.
• Minimum 2 years in healthcare with remote support preferred.
• Fluent in English, both spoken and written.
• Experience with telehealth or remote care delivery.

Responsibilities

• Guide participants through the Blue Circle Program and build relationships.
• Educate participants about the Blue Circle Health T1D program.
• Coordinate care activities, track appointments, and facilitate services.
• Assist in setting up Zoom and understanding program technology.
• Monitor progress and support care plan adjustments with the Care Team.
• Maintain accurate EHR documentation of interactions and plans.
