Job summary

Beautiful Minds Group LLC is hiring an experienced Case Manager to oversee individualized care plans for individuals with developmental disabilities in Ohio. You will coordinate ISP meetings, advocate for clients, and ensure compliance with DODD standards across multiple sites.

The role requires 3–5 years in the DD field, strong organizational and communication skills, and valid Ohio credentials including a driver’s license.

Qualifications

• Minimum 3–5 years of direct experience in the DD field as Lead DSP, Site/Residential Manager.
• Thorough understanding of Ohio DODD guidelines and regulations.
• Experience developing, implementing, and monitoring ISPs for multiple clients/sites.
• Exceptional organizational, interpersonal, and communication skills.
• Valid Ohio driver's license, reliable transportation, and current auto insurance.
• Ability to pass a criminal background check per Ohio DODD standards.
• CPR/First Aid and 8hr DSP Certification and Medication Administration recognized by Ohio DODD.
• High School Diploma or Equivalent.

Responsibilities

• Oversee daily operations between site management and staff to ensure quality care of assigned clients.
• Coordinate and facilitate ISP meetings with individuals, families, guardians, SSA representatives, and care team members.
• Serve as a primary advocate for assigned individuals, ensuring needs, preferences, and rights are respected.
• Monitor service delivery and ensure compliance with Ohio DODD rules for multiple sites.
• Maintain accurate documentation and records per Beautiful Minds Group policies and Ohio DODD standards.
• Provide crisis intervention and problem-solving support as needed.
• Participate in ongoing professional training to stay informed of best practices and regulatory requirements.
