Qualifications

• Active, unrestricted RN license with no disciplinary action.
• 5+ years of RN experience.
• 3+ years in case management.
• 1+ years of data-driven leadership experience.
• Strong computer proficiency in Microsoft Office and ability to learn multiple software apps.

Responsibilities

• Provide leadership and coaching for telephonic, Stars-focused member outreach.
• Identify member needs beyond outreach and refer to appropriate resources.
• Set performance expectations and manage outreach, care-gap closure, and outcomes.
• Translate Stars priorities into clear team actions and monitor results.
• Collaborate with enterprise teams to improve performance and impact.
