Qualifications

Responsibilities

• Collaborates with nursing leadership and pharmacy team to coordinate care.
• Review intakes, draft referrals, and arrange nursing care for patients.

• Organize and share patient care information across staff.
• Ensure documentation within EMR systems is accurate and up-to-date.
• Maintain compliance with external standards and regulations.
• Serve as liaison between interdisciplinary teams.
• Set up RN work schedules to meet staffing needs.
• Maintain daily staffing reports and provide weekly updates.
