Job summary

SYSTEM West Virginia University Health System is seeking a licensed clinical social worker to provide psychosocial assessments and treatment planning within a remote, patient-centered care model. You will participate on interdisciplinary teams, deliver crisis intervention, and support discharge planning for patients and families.

Candidates must hold a Master’s in Social Work, be licensed in WV (LGSW/LCSW/LICSW) or eligible; MD, PA, OH licensure also noted.

Qualifications

• Master’s degree in social work is required.
• Two years of social work experience preferred.
• Current licensure in the state where services are performed; multiple state licenses noted.
• Certification in Case Management is a plus.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Participate as part of an interdisciplinary treatment team.
• Develop and implement comprehensive treatment plans with evidence-based interventions.
• Screen and provide therapy for various conditions, including substance use and mood disorders.
• Educate patients and families, coordinate with care teams, and facilitate discharge planning.
