Qualifications

• Master’s degree in social work or related field.
• Current social worker licensure as required by the state where services are being performed.
• Two years of social work experience.
• Certification in Case Management (preferred).
• PA: No state licensure required; licensure varies by state per policy.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and community strengths and problems related to their diagnosis, illness, treatment, and life situation.
• Participate as an integral professional in an interdisciplinary treatment team led by physician.
• Formulate, develop, and implement comprehensive treatment plans utilizing appropriate and evidence-based treatments and interventions.
• Screens, educate on diagnosis, and provide therapy for various problems including substance abuse, depression, and anxiety.
• Provide consultative services to health care team members within scope of care definitions.
• Provide education to patients and families around adaptation to diagnosis, illness, and treatment.
• Participate in multi-disciplinary health care teams and liaison with patient and family.
• May assist with discharge planning starting on admission.
• Acts as a liaison with community resources to assist patients and families.
• Facilitate cognitive/psychoeducational group therapy and individual therapy as ordered.
• Provide updates to families and care providers and coordinate family meetings.
• Facilitate referrals to placement options and document all interactions per policy.
• Maintain EMR documentation and participate in meetings.
