Job summary

Qualifications

• Master's degree in social work or related field is required.
• Current social worker licensure as required by the state where services are performed.
• Two years of social work experience.

Responsibilities

• Provide psychosocial assessments of patients and families and identify strengths and problems.
• Participate in an interdisciplinary treatment team led by a physician.
• Formulate and implement comprehensive, evidence-based treatment plans.
• Screen and educate on diagnosis; provide therapy for various presenting problems.
• Provide consultative services and stay informed on medical/legal issues impacting care.
• Educate patients and families on adaptation and discharge planning; liaison with resources.
• May facilitate group therapy and assist with discharge planning.
