Job summary

Sentara Health is seeking an Integrated Case Manager, RN. This remote position requires residing in Suffolk, Norfolk, VA Beach, Portsmouth, VA and nearby areas with travel up to an hour to visit members, roughly 50–75% of the week.

The role involves case management across physical and behavioral health, social services and long-term care, building care plans and coordinating with the care team to optimize outcomes and reduce costs.

Qualifications

• Associates Degree required
• Bachelors preferred
• Registered Nurse required
• 3 years nursing experience required
• Discharge planning experience preferred

Responsibilities

• Perform telephonic or face-to-face clinical assessments to identify, evaluate, and manage member needs across physical, behavioral health, social services and long-term care.
• Develop and monitor care plans to optimize health care across the care continuum.
• Coordinate care with the member and health care team; identify high-risk members and manage illnesses and comorbidities.
• Facilitate authorizations/referrals within benefits or extra-contractual arrangements and interface with medical leadership.
