Job summary

Bon Secours is recruiting a Remote Social Worker to provide clinical care management for identified eligible patients across the Hampton Roads region. You will coordinate care, develop comprehensive care plans, and address barriers to access using community resources.

Minimum 2–3 years in acute care, home health, or case management with a Bachelor’s in Social Work; licensure as required by state and CMS-certified case management experience preferred.

Qualifications

• Bachelor's Degree required.
• Bachelor's or Master’s in Social Work preferred.
• Licensure as required by state of practice; Case Management certification (LSW/LCSW) preferred.

Responsibilities

• Maintain a caseload per department policies.
• Identify, enroll and manage patients in Complex Case Management.
• Develop and implement care plans with periodic reviews.
• Collaborate with ACM, PCPs, Specialists and Hospitalists.
• Assess social determinants of care and set goals in the care plan.
• Document communications in the EMR.
• Coordinate services for post-acute or long-term care as needed.
