Job summary

SCAN Group in the United States (remote-friendly) seeks a qualified Behavioral Health Care Manager to support high‑risk members with serious mental illness, SUD, and psychosocial barriers. You will coordinate telephonic, virtual, or in-person visits and collaborate with the care team to assess and implement holistic care plans.

The role emphasizes patient-centered care, coordination with BH/MH vendors, and Enhanced Care Management for focus populations.

Qualifications

• Graduate or Advanced Degree required in Social Work.
• Licensed Clinical Social Worker preferred; CCM preferred.
• 3-5 years managed care, healthcare environment, or mental health case management.
• 2 + years working with seniors and working remotely strongly preferred.
• Technical expertise - Advanced analytical skills
• Problem Solving - Basic problem-solving skills
• Communication - Good communication and interpersonal skills
• Bilingual Spanish/English strongly preferred. A test will be administered to assess proficiency if applicable.
• Strong interpersonal skills, including excellent written and verbal communication skills.
• Strong organizational skills.
• Strong critical thinking skills.
• Ability to collaborate effectively with a multidisciplinary team.
• Ability to appropriately maintain confidentiality.
• Ability to communicate complex medical information with simplicity and kindness and build strongrapport.
• General understanding of NCQA standards, CMS and DHCS regulations

Responsibilities

• Conduct comprehensive assessments to identify chronic conditions and psychosocial challenges affecting independent living.
• Manage and provide targeted support for high‑risk members with serious mental illness, substance use disorder, and behavioral barriers.
• Ensure care plans are clinically appropriate by evaluating findings against guidelines.
• Provide education and coaching on behavioral and mental health treatments.
• Collaborate with BH/MH vendors and programs; act as SME for complex cases.
• Engage members to activate participation and communicate plans of care.
• Perform Enhanced Care Management for populations of focus such as homelessness and avoidable hospital use.
• Participate in interdisciplinary planning and discharge follow-up.
• Adhere to quality, compliance, and regulatory standards.
