Qualifications

• Requires a bachelor’s degree in social work, psychology, or related field with practicum experience.
• Alternative: at least three years of case management experience in social services or healthcare setting.

Responsibilities

• Develop a person-centered service plan addressing identified needs, goals, interventions and progress.
• Coordinate services for participants based on identified needs.
• Monitor authorized services via in-home visits, phone calls, and planning meetings with providers.
