Job summary

Alleghenies United Cerebral Palsy is seeking a Service Coordinator to develop and manage personalized service plans with clients and support teams. You will oversee paid providers and community resources to ensure services meet needs and comply with regulations.

This hybrid/remote position requires PA residence or willingness to travel for home visits, with strong communication and coordination skills essential for success.

Qualifications

• Bachelor's degree in Social Work, Psychology, Education, Human Services, or related field.
• At least 3 years of professional case management experience for people with physical disabilities.
• 3+ years of experience working with individuals with Autism.

Responsibilities

• Collaborate with clients and teams to develop individualized service plans (ISP).
• Regularly update service plans to reflect changes in needs.
• Coordinate and monitor services with paid providers and community resources.
• Maintain open communication with support teams to address concerns promptly.
• Ensure compliance with program regulations, documentation, timelines and billing.
