Qualifications

• Bachelor’s degree in social sciences or 12 credits plus 2 years related experience.
• Ability to work independently and manage a dynamic caseload.
• Strong interpersonal, organizational, and communication skills.

Responsibilities

• Assess individual strengths, needs, and life goals.
• Develop person-centered plans with the individual and their circle of support.
• Coordinate services aligned with goals and ensure timely access to supports.
• Monitor service delivery and adjust supports to remain effective.
• Document activities and communications per regulatory standards.
• Support families with tools to promote informed decision-making.
• Engage in ongoing professional development and training.
