You will oversee medication therapy, lead comprehensive medication reviews, and optimize prescriptions for patients taking multiple medications. Qualified candidates hold a PharmD or BS in Pharmacy with an active pharmacist license, strong organization, problem-solving, and the ability to work independently and with teams.

Responsibilities

• Oversee medication therapy and strategy for comprehensive medication reviews

• Lead Rx education and quarterly campaigns with patient outreach
• Influence department strategy and make decisions on moderately complex issues
• Counsel patients and provide clear communication across the organization
