Job summary

Qualifications

• Requires a Master’s degree in Behavioral Health or Social Work or a nursing degree with BH experience.
• Licensed Master’s Behavioral Health Professional (LCSW/LMSW/LMFT/LMHC/LPC) or RN based on state contract requirements.

Responsibilities

• Develops and facilitates complex care management activities for behavioral health needs.
• Evaluates member needs via phone or home visits and coordinates care plans.
• Identifies providers, resources, and community services for timely access to care.
• Provides education to members and families about procedures, benefits, and treatment options.
