Job summary

SCAN Health Plan is seeking a Medical Director to lead clinical teams delivering high-quality, home-based care across multiple states. You will oversee geriatrics and palliative care, plus telehealth initiatives, with a focus on outcomes and regulatory compliance.

The role offers a base salary and quarterly bonus, with a remote/field work mode and generous benefits to support a mission-driven organization serving older adults.

Qualifications

• Must hold Doctoral Degree M.D. and unrestricted medical license.
• Geriatrics/palliative care experience of 5+ years preferred.
• Experience with virtual care/telehealth is a plus.
• Knowledge of CMS guidelines and Medicare Advantage is expected.
• Familiarity with ICD-10/HCC coding and EMR systems is valued.

Responsibilities

• Lead a multi-disciplinary team to ensure coordinated, high-quality care.
• Track and analyze clinical performance metrics and utilization.
• Identify improvement opportunities and implement care-enhancing strategies.
• Provide primary medical care to home-based patients and conduct rounds both in person and virtually.
• Maintain EMRs and ensure regulatory compliance.
