Sagility is seeking a Senior Care Specialist (RN) to work from home in the United States. The role focuses on telephonic health assessments, care planning, and coaching for Medicare/Medicaid members, with a flexible seasonal assignment ending June 2027.

Candidates must hold an active RN license and Compact license where applicable, and be proficient with MS Office in a remote setting. The position offers competitive hourly pay and the possibility of daily pay.

Competenze

• Active, unrestricted RN license in your state.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years clinical nursing experience; 3+ years preferred.
• Ability to work independently and exercise nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including MS Word, Excel, and Outlook.

Mansioni

• Conduct telephonic health assessments for members.
• Develop actionable, individualized care plans based on health risks.
• Establish health coaching relationships with members and provide education and support.
• Use clinical nursing judgment to assess member needs and coordinate interventions.
• Coordinate care and transitions, including DME, home health services, and appointments.
• Document member interactions and outcomes accurately in applicable systems.
