Job summary

Qualifications

• RN with case management experience preferred.
• Current WA RN license.
• CCM/CIRS certification preferred.
• Willingness to travel locally up to 60%.
• Excellent communication and collaboration skills.

Responsibilities

• Provide in-person and telephonic Medical Case Management to patients, families, providers and employers.
• Discuss treatment plans with physicians and discuss alternatives.
• Assess, plan, implement, and evaluate patient progress.
• Evaluate treatment plan for medical necessity and cost-effectiveness.
• Travel to assignments and participate in discharge planning conferences.
• Coordinate delivery of equipment and nursing services.
• Travel up to 60% of time as needed.
• Perform additional duties as required.
