Job summary

The CKHobbie Group is seeking a qualified RN to serve as a high-risk maternity case manager for Medicaid members. This remote role supports residents in Southeastern PA, delivering care coordination, health education, and community resource connections.

You will conduct member assessments, develop individualized care plans, and report outcomes in team rounds with the Medical Director, while ensuring culturally competent education and collaboration with a small care team.

Qualifications

• Active, unrestricted PA RN license.
• 3–5 years of nursing experience in high-risk maternity care.
• Experience in obstetric or maternal-fetal medicine settings.
• Strong verbal and written communication skills.

Responsibilities

• Serve as high-risk maternity case manager for Medicaid members.
• Provide care coordination, health education, and community resource support.
• Conduct member assessments and develop personalized care plans.
• Document cases for state reporting and participate in team rounds with Medical Director.
• Engage with members via phone or video with a culturally competent approach.
