Job summary

Alliance Health is seeking a Care Manager for the Care Management for High-Risk Pregnancy program. The role focuses on identifying risks, coordinating care, and supporting pregnant members through postpartum transition with a whole-person approach.

Responsibilities include outreach, assessments, care plan development, and collaboration with providers and community partners. Employment requires NC licensure and relevant care management experience; hybrid/remote options are available.

Qualifications

• Master’s degree and at least two years in care management or related field.
• Licensed in NC as LCSW/LMFT/LCAS/LCMHC/LPA or RN with three years post-degree experience.
• Preferred: experience in maternal health, Medicaid managed care, or related services.

Responsibilities

• Provide care team support for CMHRP during pregnancy and postpartum.
• Develop and maintain member-centered care plans with providers and supports.
• Coordinate with OB/PMP providers, hospitals and community partners.
• Document outreach, assessments, referrals and care activities accurately.
• Identify barriers to care (transportation, housing, food security, language access) and escalate when needed.
• Travel between Alliance offices and meet with members and providers as required.
