Job summary

Molina Healthcare is seeking an experienced RN to serve as an IRIS Self-Directed Personal Care (SDPC) nurse in Wisconsin. This remote role partners with individuals in their homes and communities enrolled in the IRIS program, providing oversight, education, and person-centered care planning.

The SDPC RN administers screening tools, develops care plans, and performs required documentation to ensure optimal support for adults with disabilities and aging individuals.

Qualifications

• Two years of nursing experience with one year in target IRIS groups or equivalent.
• Active RN license in Wisconsin and valid driver's license required.
• Proficient with Microsoft Office and data-entry tasks.

Responsibilities

• Administer Wisconsin Personal Care Screening Tool and addendums as required.
• Develop and oversee individual plans of care for IRIS participants.
• Provide education and training to participants and care providers.
• Complete required documentation and follow-up activities.
• Collaborate with IRIS Consultants and physicians to meet care needs.
• Perform home visits and related driving as needed.
