Job summary

Molina Healthcare seeks a Registered Nurse licensed in Wisconsin for a remote IRIS Self-Directed Personal Care (SDPC) RN position. You will support IRIS participants in their homes, provide oversight, training, and education, and ensure comprehensive care plans are implemented effectively.

You will conduct assessments, develop person-centered plans, and ensure documentation and compliance with DHS policies. This role requires travel up to 50% and home-based visits across the community.

Qualifications

• At least 2 years nursing experience and at least 1 year serving IRIS program participants or equivalent.
• Active and unrestricted RN license in Wisconsin.
• Associate's degree in nursing required.

Responsibilities

• Oversee and guide IRIS SDPC participants with person-centered care plans.
• Administer Wisconsin Personal Care Screening Tool and required addendums.
• Develop and revise care plans, obtain physician orders, and monitor care quality.
