Job summary

Molina Healthcare is seeking an RN licensed in Wisconsin for a remote IRIS Self-Directed Personal Care (SDPC) role. You will oversee care for individuals in the IRIS program, conducting assessments and developing person-centered plans from a home-based, community-focused setting.

Qualifications

• At least 2 years nursing experience and 1 year serving IRIS target groups
• Active Wisconsin RN license
• Associate's degree in nursing
• Valid driver's license and reliable transportation for job-related travel
• Database operation/maintenance and data entry experience
• Teaching and mentoring skills
• Strong written and verbal communication, organizational and time-management skills
• Flexibility to adapt to changing organizational needs
• Proficiency with Microsoft Office or applicable software

Responsibilities

• Administer Wisconsin Personal Care Screening Tool and oversight visits
• Develop and review individual plans of care; obtain physician orders as needed
• Provide personal care training to participants and care providers
• Submit for prior authorization for personal care services
• Maintain compliance with Department of Health Services policies and SDPC guidelines
• Complete collateral contacts with IRIS Consultants and care providers
• Complete required documentation and follow-up
• Overtime may be required
• Possibility of driving up to 50% of days for member visits
• Exposure to member homes and varying environments including pets
