Job summary

Molina Healthcare seeks a Registered Nurse licensed in Wisconsin for an IRIS Self-Directed Personal Care RN role. This remote position partners with IRIS participants in their homes and communities, providing oversight, assessment, and education to ensure high-quality self-directed care.

You will build relationships with participants, create plans of care, and document outcomes in accordance with DHS policies. Overtime and travel may be required.

Qualifications

• At least 2 years nursing experience and 1 year in IRIS program or equivalent.
• Active and unrestricted RN license in Wisconsin.
• Associate's degree in nursing.
• Valid driver's license and reliable transportation for travel.
• Experience with data entry and database maintenance.
• Strong teaching, mentoring, and communication skills.
• Excellent organizational and time-management abilities.
• Flexibility to adapt to changing needs.

Responsibilities

• Provide oversight to IRIS SDPC participants and administer care assessments.
• Develop and revise person-centered care plans and obtain physician orders.
• Submit authorizations for personal care services and ensure policy compliance.
• Deliver training and education to participants and care providers.
• Document visits and interactions; perform collateral contacts as needed.
