Job summary

Privia Health is seeking an RN for our remote Care Advice Line (CAL RN1) to join the Population Health team. You will provide on-demand telephone-based care management and 24/7 nurse triage, coordinating with providers and scheduling appointments as needed.

The role emphasizes HIPAA compliance, strong communication, and the ability to work from a quiet home office; shifts include weekday evenings and weekend coverage with flexible 2,4,8,12-hour options.

Qualifications

• 1+ years of RN experience in adult primary care and/or pediatric care in office, clinic, military, home, urgent care or hospital setting.
• BSN preferred or willingness to obtain within 24 months.
• Compact Resident RN in good standing, with the ability to apply to non-compact states as needed (expenses reimbursed).
• Availability to work 10-30 hours per week, including at least 10 hours on weekends (3 out of 4) and every 3rd holiday.

Responsibilities

• Handle inbound calls from patients and caregivers, providing triage.
• Conduct outbound communications for follow-up and care coordination with providers.
• Coordinate with team members to assess, plan, implement, and evaluate patient care plans.
• Assist with finding appropriate providers and care solutions and coordinate virtual appointments.
• Record member data in Privia’s EMR and health portals.
• Maintain accurate patient records in Privia's systems.
