Remote RN Field Care Coordinator

Job summary

UnitedHealth Group is hiring a Care Coordinator (RN) to coordinate member care needs for medically complex patients within Monroe County and surrounding areas. This field-based role requires strong clinical knowledge, documentation, and collaboration with the care coordination team.

You will perform assessments, develop care plans, and coordinate services to ensure access to care while maintaining regulatory compliance. Remote work flexibility is allowed within commutable distance.

Qualifications

• Current Nursing License (RN)
• 2+ years of clinical experience as an RN
• 1+ years of experience with MS Office (Word, Excel, Outlook)
• Excellent organization, communication and time management skills
• Reside in or within commutable distance to Monroe County
• Ability to travel up to 75% within assigned territory

Responsibilities

• Apply clinical knowledge to coordinate case management for Medicare/Medicaid beneficiaries
• Incorporate evidence-based practices into care coordination
• Provide member-centered, self-management support and education
• Ensure appropriate utilization and benefit application
• Conduct telephonic and in-person assessments per policy and timeframes
• Advocate for members and coordinate community resource referrals
• Develop and maintain member-centered care plans
• Collaborate with Care Coordination team to improve care quality and efficiency
• Monitor hospital encounters and provide education/interventions as needed
• Refer cases beyond authority to manager for review
• Participate in Health Plan Quality Improvement activities
• Document all care coordination activities in Health Plan systems
• Maintain time-sensitive documentation to ensure regulatory compliance
• Coordinate services per Health Plan policy
• Perform other duties as assigned
