Job summary

UnitedHealth Group seeks a Discharge Care Manager in Arizona to coordinate post-discharge care for high‑risk members by telephonic case management and collaboration with clinicians. You will document discharge plans and monitor patients after hospital discharge to prevent delays and readmissions.

Required: Arizona RN license, 5+ years inpatient experience, 2+ years in discharge planning or case management; proficiency with MS Office; remote work options available for AZ residents; salary range

Qualifications

• Current, unrestricted Compact RN license in Arizona.
• 5+ years of recent experience in the acute inpatient hospital setting.
• 2+ years of discharge planning and/or case management.
• Experience with Medicaid/Medicare and other insurance products.
• Proficient in MS Word, Excel, PowerPoint and Outlook.
• Home workspace with secure high-speed internet; reside in Arizona.

Responsibilities

• Evaluate discharge needs and develop readmission prevention plans.
• Coordinate post-discharge care with providers and members.
• Attend rounds with Medical Director to discuss cases.
• Identify community resources and program support.
• Document discharge activities per SOPs and data entry guidelines.
• Collaborate with care teams to ensure end-to-end patient experience.
