Job summary

UnitedHealth Group is seeking a Discharge Care Manager to execute telephonic case management for high-risk members. You will assess needs, coordinate discharge plans with clinical teams, and support post-discharge care across the care continuum.

Requires a BSN and an active Florida or Massachusetts RN license, with 5+ years inpatient experience and 2+ years in discharge planning or case management. Remote work is available for FL/MA residents.

Qualifications

• BSN required.
• Current, unrestricted Compact RN license in FL or MA.
• 5+ years of inpatient hospital experience.
• 2+ years in discharge planning or case management.
• Experience with Medicaid/Medicare/Commercial insurance products.

Responsibilities

• Evaluate discharge needs and plan post-discharge care.
• Coordinate care with providers and care teams after discharge.
• Participate in rounds with Medical Director to discuss cases.
• Document discharge activities and maintain SOP/data-entry accuracy.
• Collaborate with discharge planners to arrange post-hospital services.
