Job summary

Yale New Haven Health is seeking a Clinical Triage Nurse to join a fast-paced call center and provide patient-centered triage using protocols and standing orders to guide care.

You will communicate with patients, families, and providers, collaborate with clinical practices, and maintain accurate documentation of all clinical activity while upholding confidentiality and quality standards.

Qualifications

• Graduate of an accredited school of Nursing.
• RN license in CT required.
• BLS and ACLS per departmental policies.
• 3 years of clinical experience in direct patient care.
• Triage or ICU/ED experience preferred.

Responsibilities

• Perform telephonic triage for a diverse patient population using clinical protocols and standing orders to direct to appropriate care.
• Collaborate with a wide array of healthcare clinicians to secure time-sensitive responses and aim for a one-call resolution when possible.
• Assess patient needs and guide next steps through effective communication with providers, patients, and families.
• Follow up with patients providing test results, education, escalation or triage for medical management of symptoms when appropriate.
• Apply clinical assessment and communication skills while adhering to established clinical guidelines.
• Document all clinical activity accurately and in a timely manner in the electronic record.
• Adapt to changing priorities and maintain flexibility to meet current needs.
• Identify opportunities for quality improvement to enhance patient value.
• Maintain strict patient confidentiality.
• Expand knowledge of regional resources to help patients access appropriate services.
• Work independently while maintaining nursing competencies.
• Obtain all requested clinical documentation.
• Coach and mentor new triage staff and provide routine feedback.
• Demonstrate patient service excellence at all times.
• Build positive relationships with patients and clinical teams to support workflow and protocols.
