Job summary

Yale New Haven Health is seeking a Transitional Care Manager Triage RN to join the Enterprise Office of Population Health's Ambulatory Care Management team. You will provide clinical triage, post-discharge follow-up, and care coordination to improve health outcomes and reduce readmissions.

The role requires RN licensure in Connecticut, a Bachelor's degree in a clinically related field, and at least 3 years of clinical experience with 2 years in case management.

• Generate and analyze daily EHR reports to support care transitions.
• Educate patients and families on discharge instructions and disease management.
