Lumicera Health Services is seeking a skilled RN for a remote, telephonic education role. You will educate patients about specialty therapies, assess clinical needs, and support adherence and safety under approved protocols.

The position emphasizes accurate documentation, collaboration with pharmacists, and adherence to regulatory guidelines to improve patient outcomes. Remote work across the US is available.

Formación

• ADN required; BSN preferred.
• Current, unrestricted RN license in home state.
• 2 years patient-centered care or disease management experience.

Responsabilidades

• Conduct clinical assessments and provide disease- and therapy-specific education.
• Identify and address patient needs related to therapy use, adherence, persistence, adverse effects, and disease concerns; triage to pharmacist or notify provider when appropriate.
• Document clinical assessments, interventions, care coordination, and notes in patient records per requirements.
• Support planning, implementation, and maintenance of clinical programs and projects to improve patient care and outcomes.
• Complete required documentation, including adverse event forms and program-specific forms, per requirements.
• Collaborate with pharmacists, clinical team members, manufacturers, and providers to support programs and patient care.
• Assist with order creation and setup activities to support patient and business needs.
• Maintain clinical competency across treatments and disease states supported by Lumicera.
• Ensure compliance with accreditation, policies, HIPAA, and regulations.
