Job summary

Lumicera Health Services is seeking an RN, Clinical Operations to provide telephonic patient education and clinical assessment for specialty therapies. This remote role supports adherence, outcomes, and timely escalation when needed.

You will document interventions, coordinate with pharmacists and providers, and ensure compliance with HIPAA guidelines while maintaining high-quality patient care.

Qualifications

• ADN required; BSN preferred.
• Current RN license in state of residence; multi-state licensure preferred.
• 2 years of patient-centered care or disease management nursing experience preferred.

Responsibilities

• Conduct clinical assessments and provide disease- and therapy-specific education to patients and caregivers.
• Identify patient needs related to therapy use, adherence, persistence, adverse effects, and escalate when appropriate.
• Accurately document assessments, interventions, and care coordination in the patient record per requirements.
• Support planning, implementation, and maintenance of clinical programs to improve outcomes.
• Complete required documentation including adverse event forms per regulatory requirements.
• Collaborate with pharmacists, clinicians, and providers to support patient care needs.
• Assist with order creation and setup activities to support patient needs.
