Salary: $90,000 - $110,000 per year
Job Details
Reporting to the Supervisor of Care Management, this position will provide MLTC case management to our members including medical, social, psychological, physical, and spiritual needs.
RN Case Manager will develop, implement, and monitor the care plan, in conjunction with the PCP, caregivers, and other team members, to help meet the member's needs.
This is a REMOTE RN Case Manager role and REQUIRES the RN to live in the NYC Metro area to be considered for employment.
RN must have an active New York State RN license in good standing.
RN will be required to TRAIN in the NYC office for the first 3 months of employment and will then transition to telecommute after successfully completing the 3-month probation period.
RNs who have Managed Care or MLTC Case Management experience will be highly considered.
