Job summary

EK Health is hiring a Telephonic Nurse Case Manager (RN) for our Case Management Team. You will assess, plan, implement, coordinate, and evaluate service options to help injured workers receive timely, cost-effective care and expedite their return to work.

Position is full-time remote, Monday–Friday, 8:30am–5:30pm local time. Requires a Compact RN license in good standing; CA RN license within 90 days is reimbursed.

Qualifications

• Graduate of an accredited school of nursing.
• 3–5 years clinical RN experience outside of school.
• Compact RN license in good standing with no restrictions.
• Current state RN license in good standing with no restrictions.
• Must obtain CA RN License within 90 days (reimbursed).
• Experience in Case Management; Workers' Compensation preferred but not required.
• Experience in Home Health Care; Occupational Health a plus.
• Excellent written and oral communication skills.
• Strong interpersonal and organizational skills.
• Proficient with computers (MS Word, MS Excel).

Responsibilities

• Communicate with medical providers, employers and with injured workers.
• Perform a complete nursing evaluation to determine patient needs.
• Review and evaluate medical correspondence, provider reports, and treatment plan history.
• Evaluate claimant clinical status and explore alternative treatment options.
• Coordinate with claims examiners and provide updates on file status.
• Arrange transportation services when necessary and authorized.
• Evaluate therapy facilities and track case progress.
• Prepare comprehensive notes from discussions with involved parties.
• Discuss plans with the insurance representative prior to implementation.
• Implement plan of care with patient, physician and providers.
• Arrange care/services (home care, procedures, medications, equipment).
• Monitor and adjust the plan of care as needed.
• Coordinate information among all parties involved.
