Qualifications

• Active RN license with 5+ years experience, including 1 year in a hospital setting.
• Multistate license via NLC or state-specific licenses for each state supported.

Responsibilities

• Perform telephonic case management with patients and care teams.
• Assess benefits and needs; apply guidelines to determine appropriate benefits.
• Address complex health indicators in care planning and reduce risk factors.
• Collaborate with supervisors and medical directors; present cases in conferences.
• Use motivational interviewing to engage members and assess health needs.
• Identify and escalate member needs per protocols.
• Actively reach out to guide members’ care and conduct medical necessity reviews.
