Job summary

CVS Health is seeking an experienced RN Case Manager to work remotely nationwide. You will perform telephonic case management for (fully or self-funded) clients, applying clinical guidelines and plans to ensure appropriate benefits and high-quality care.

Requirements include 5+ years as an RN with hospital experience, an active multistate license (NLC) or state licenses, and oncology exposure. The role offers full-time hours and remote work across the US.

Qualifications

• 5+ years as a Registered Nurse with hospital experience.
• Active multistate license via NLC to practice across participating states.
• 1+ years documenting electronically.
• 1+ years' oncology experience.

Responsibilities

• Telephonic case management with patients and care teams for fully/self-insured clients.
• Assess benefits and needs using criteria, guidelines, policies and regulatory standards.
• Apply clinical judgment to reduce risk factors and address barriers in care planning.
• Utilize information from multiple sources to address comorbid conditions.
• Consult with supervisors and medical directors to overcome barriers and meet goals.
