Job summary

CVS Health is seeking a Telephonic RN Case Manager to support fully and self-insured clients from a remote location across the US. You will apply clinical guidelines, coordinate care and advocate for members while collaborating with care teams and supervisors.

The role requires 5+ years as an RN (including hospital experience), a multistate license via NLC or individual state licenses, and oncology experience. This full-time position offers 40 hours weekly and a comprehensive benefits package.

Qualifications

• 5+ years' experience as a Registered Nurse, including at least 1 year in a hospital setting.
• Active multistate license via NLC or state-specific RN license for each state served.
• 1+ years' experience documenting electronically.
• 1+ years' experience in Oncology.

Responsibilities

• Telephonic case management with patients and care teams.
• Assess benefits and patient needs to ensure appropriate administration of benefits.
• Apply clinical judgment to strategies to reduce risk factors.
• Address complex health and social indicators impacting care planning.
• Collaborate with supervisors and other programs to overcome barriers.
• Utilize case management processes in compliance with policies.
• Engage members using motivational interviewing to assess health status.
