Job summary

CVS Health is seeking a licensed RN Case Manager to work remotely across the United States, conducting telephonic case management for fully insured and self-insured clients with clinical judgment and collaboration with care teams.

The role requires 5+ years as a RN with oncology experience, a multistate license or state licenses, a Diploma or Associate Degree in Nursing, and 40 hours weekly of full-time work. Competitive pay and benefits are offered.

Qualifications

• 5+ years' experience as a Registered Nurse, including at least 1 year in a hospital setting.
• RN must have active multistate license through NLC to practice across participating states or individual state licenses for non-compact states.
• 1+ years' experience documenting electronically using a keyboard.
• 1+ years' current or previous experience in Oncology.

Responsibilities

• Telephonic case management with patients and care teams for fully and/or self-insured clients.
• Apply criteria and clinical guidelines to assess benefits and member needs for appropriate benefits administration.
• Use clinical judgment to reduce risk factors and address barriers in care planning.
• Assess information from multiple sources to address all conditions including co-morbidities.
• Consult with supervisors and colleagues to overcome barriers and discuss cases in conferences.
