Job summary

CVS Health is seeking an experienced RN Case Manager for a remote role across the United States. The position involves telephonic case management of members, applying clinical guidelines, and coordinating care with care teams to optimize outcomes.

Qualifications

• RN license with multi-state eligibility per NLC.
• Active RN license for states supported; compact state license preferred.
• 1+ year of documenting electronically using a keyboard.
• 1+ year of Oncology experience.

Responsibilities

• Conduct telephonic case management with patients and care teams for fully and/or self-insured clients.
• Apply criteria, guidelines, policies, and regulatory standards to assess benefits and member needs.
• Use clinical judgment to reduce risk factors and barriers in care planning.
• Assess conditions including co-morbidities and multiple diagnoses affecting functionality.
• Consult with supervisors and colleagues to overcome barriers and meet goals.
• Maintain compliance with case management processes and policies.
