Qualifications

Responsibilities

• Telephonic case management with patients and care teams for clients.
• Apply clinical guidelines and care management plans to assess benefits and member needs.
• Identify barriers and risk factors and design care strategies.
• Consult with supervisors and multidisciplinary teams to meet goals.
• Use case management processes in compliance with regulatory standards.
• Engage members to maximize engagement and adherence to care plans.
• Perform medical necessity reviews.
