Job summary

Elevance Health is seeking a Nurse Case Manager I to perform care management for members with complex and chronic needs. The role supports telephonic and on-site activities, with occasional in-person training and a requirement to be near a posting location.

Qualifications

• Requires BA/BS in a health-related field and 3+ years of clinical experience.
• Current, unrestricted RN license in applicable state(s). Multi-state licensure required for multi-state services.

Responsibilities

• Provide care management across the care continuum.
• Assess, develop, implement, coordinate and monitor care plans.
• Coordinate authorizations/referrals within benefits or extra-contractual arrangements.
• Interface with Medical Directors/Physician Advisors on care plans.
• Prospect and negotiate appropriate reimbursement where applicable.
